At times, a veteran’s care needs may become more complex and require a higher level of coordination and management. That’s when VeteranCare can match our veteran clients with a Professional Care Manager–typically an experienced Nurse or Social Worker, who can assist them to navigate these challenges.
The Care Manager will conduct a professional assessment, evaluating medical and psycho-social history and current concerns. They will create a Care Plan – a roadmap and action plan to optimize the care of the veteran and/or spouse.
A Care Manager is knowledgeable regarding local resources such as physicians, hospitals, therapists, and rehabilitation facilities. A Professional Care Manager can help the veteran, spouse, and family access all the services necessary to stay at home independently with support, or alternatively help them safely transition to an appropriate Assisted Living Facility, if and when it becomes necessary. A Care Manager serves our veterans and spouses as trusted advisors to optimize their care.
